You can use this service if you:

• are registered at the surgery
• have been invited to do so

Important information

• You will need to complete this form every month
• You will receive a text message when your next form is due
• Your GP will review your responses. If it is safe and appropriate to continue treatment, your prescription will be sent to your nominated pharmacy
• If you do not respond, this may affect your ability to continue treatment

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
